RhinoCam Mill/Turn capability - is it available?

In Camjam there are tutorials for Mill/Turn operations. I have RhinoCam-STD, which includes Mill and Turn packages, but not Mill/Turn...
Neither is Mill/Turn listed as a purchasable package option. How do I acquire Mill/Turn CAM functionality, and where do I find a description as to how it works and what machine capabilities are required? Does it include C axis or is it only a separate mill axis with the lathe spindle as an indexable rotary axis?
