What am I doing wrong

I have been working on this for quite a while, and I am not able to get a decent result. This is after a horizontal roughing, and two parallel finishing passes 90º to each other. I have tried different bits and different approaches, but this is the best I have gotten. Other attempts left raised portions around all the holes and was very rough. This is smoother, but is gouging in some spots and removing to much material in others.
