Same issue for me. What happened to the usual selections in the menu tab?

It looks like the VisualMILL 6.0 for SolidWorks did not uninstall properly. You might have been using VisualMILL 6 on SolidWorks 2012 and the older installer did not account for the 2012 product. You can run this exe to clean out the registry. Please follow this procedure:
1) Uninstall VisualMILL 2012 for SolidWorks
2) Run the RemCmdTab.exe (use the 32 or 64 bit versions as appropriate)
3) Reinstall VisualMILL 2012 for SolidWorks
This should fix the problem.

Thank you for your timely response. Following the steps in the last post did solve the problem and the interface is now showing correctly.
